What is a Primary Care Network?

A Primary Care Network (PCN) is a group of GP practices working closely together with other local health and care organisations.

Working together around local communities

PCNs bring practices together with community, mental health, social care, pharmacy, hospital and voluntary services. This helps local teams provide more proactive, personalised, coordinated and integrated care close to home.
