Musculoskeletal and First Contact Physiotherapy
Available to patients registered with all Oakfield PCN practices
Our First Contact Physiotherapists assess muscle, joint and bone problems and can provide advice, treatment plans and onward referral where appropriate.
Who this service can help
Adults with new or ongoing muscle, joint or bone problems, including back, neck, shoulder and knee pain.
How to access this service
Contact your registered GP practice and ask about a First Contact Physiotherapy appointment. A GP appointment may not be needed first.
Mental Health Practitioners
Appointments are offered across the PCN
PCN Mental Health Practitioners work alongside practice teams to offer assessment, advice and coordinated support for adults experiencing mental health difficulties.
Who this service can help
Adults who would benefit from focused mental health assessment, support or help connecting with appropriate local services.
How to access this service
Speak to a member of your GP practice team. They can explain the service and arrange a referral when it is appropriate.
Clinical Pharmacists
Available through every member practice
Clinical Pharmacists provide expert support with medicines, long-term conditions, medication reviews and questions about how treatments work together.
Who this service can help
People taking several medicines, starting a new medicine, living with a long-term condition or needing a structured medication review.
How to access this service
Ask your GP practice about an appointment with the PCN Clinical Pharmacist. The practice may also invite you for a medication review.
Pharmacy Technicians
Working across Oakfield PCN member practices
Pharmacy Technicians support safe, accurate and efficient medicines management across our practices and help resolve practical prescription queries.
Who this service can help
Patients who need practical help with repeat medicines, medication changes after a hospital stay or questions about prescription arrangements.
How to access this service
Contact your registered practice. The reception or medicines team will direct your query to the most appropriate person.
Social Prescribing Link Workers
Community appointments available across the network
Social Prescribing Link Workers take time to understand what matters to you and connect you with local activities, organisations and practical support.
Who this service can help
People who may benefit from support with loneliness, wellbeing, housing, finances, caring responsibilities, confidence or community connections.
How to access this service
Ask any member of your GP practice team for a social prescribing referral. Some local services may also accept self-referrals.
